Joshua is a patient at our Nevada Health Centers Las Vegas Outreach Clinic (LVOC). LVOC provides healthcare and case management services for patients experiencing homelessness.
At LVOC, Joshua met to our outreach caseworker, George Changpo. George assists unsheltered patients with securing housing and other basic necessities by conducting patient needs assessments and working collaboratively with our medical providers, social workers and community resource partners to link patients to housing and other resources. He also provides transportation for patients to ensure they do not miss healthcare appointments or meetings with social workers and resource providers.
“George and NVHC have helped me tremendously with my healthcare needs. Before NVHC, I had nowhere to go, my wheel chair was falling apart, and I didn’t have the basic necessities,” says Joshua. “Being at the shelter was tough, especially because you have to leave during the day which is very hard when you’re not able to walk.”
The LVOC team secured a new wheelchair for Joshua that arrived just in time as his old chair gave out. George also helped Joshua leave the homeless shelter by securing non-congregate housing where Joshua now has his own private living space.
We asked Joshua to tell us more about his experience with Nevada Health Centers and LVOC here’s what he had to say:
Tell us about George and NVHC?
George is awesome! He and the NVHC team help me stay on track with my care. The program is amazing because of all they do. What has your experience with NVHC been?
The experience with the clinic is great. The shelter did what they could but NVHC understands our situations a lot better. No surprises and they understand top to bottom what it’s like for this community.
What stands out the most?
The support NVHC provides. I feel like I am a member of the real world and it feels like I have a home again- some normality. Because of NVHC I’m headed in the right direction. I’m happy, I’m eating, I’m taking care of my health, and I have a case manager that I work with weekly to continue getting better. Thank you, NVHC!
